Your First Appointment
A comprehensive cardiovascular evaluation may take 1 to 2 hours so please set aside 2 hours for your appointment. Make sure to bring your insurance card and co-pay amount or your deductible. We accept checks and most credit cards. Please call our billing office in advance at 502-584-3204 if you have a deductible or no insurance coverage.
You will need to bring a referral to the office visit if you have Tri Care insurance. Other patients, like managed-care plan, may need to obtain authorization from your primary care physician.
Our staff is bilingual (English and Spanish). If you need a translator (and are unable to bring one with you) we must be notified at least 72 hours in advance.

We cannot evaluate a minor (under 18 years of age) without them being accompanied by a parent or designated adult.
We strongly recommend that the legal guardian and caretaker of the patient come to the appointment, as Dr. Villafañe will take time to explain the diagnosis and any other pertinent findings and recommendations.
If for a justifiable reason the legal custodian is unable to come along to the appointment then he or she is required to fill out and sign all of the forms above and send to the appoinment with the designated adult.
